COMMUNITY HEALTH WORKER

Through support from the Augusta Health, we are celebrating our first anniversary of the Community Health Worker program!

 

This program focuses on health care navigation, health education, and goal setting to improve coordination and access to care for our LGBTQ+ community. Our Community Health Worker (CHW) will work alongside adult* members of the LGBTQ+ community to identify their health-related goals and connect them with health system and community-based resources to help achieve those goals.

*individuals under the age of 18 are encouraged to reach out to UVA's Transgender Youth Health program

HEALTHCARE NAVIGATION

 Uninsured? Underinsured? Lack of transportation? Too many other barriers to accessing care?  Our CHW can help you overcome them and receive the care that you need and deserve. 

LGBTQ+ AFFIRMING REFERRALS

We know how important it is for you to trust your medical provider. Our CHW is building a referral network of LGBTQ+ competent and affirming providers in our area.  Check out our Medical Professionals page!

MEDICAL CARE COORDINATION AND CASE MANAGEMENT

Coordinating appointments and medical care plans can be challenging.  Our CHW can help! From appointment scheduling and reminders to in-person appointment support, our CHW will be there every step of the way to help you reach your health goals.

HEALTH EDUCATION

Need the most recent updates about Covid-19 in our area? Looking for information about the Monkeypox outbreak? Want to learn more about safer sex practices?  Our CHW has you covered with accurate and culturally competent health information!

SAFER SEX SUPPLIES

To help stop to spread of STIs and HIV in our community, our CHW can provide free internal and external condoms, lubricant, and dental dams for those in need.
