COMMUNITY HEALTH WORKER
Through support from the Augusta Health Foundation, we are celebrating our first anniversary of the Community Health Worker program!
This program focuses on health care navigation, health education, and goal setting to improve coordination and access to care for our LGBTQ+ community. Our Community Health Worker (CHW) will work alongside adult* members of the LGBTQ+ community to identify their health-related goals and connect them with health system and community-based resources to help achieve those goals.
Please sign up for a consultation with our Community Health Worker at the link below if you are in need of any of these services!
*individuals under the age of 18 are encouraged to reach out to UVA's Transgender Youth Health program
Uninsured? Underinsured? Lack of transportation? Too many other barriers to accessing care? Our CHW can help you overcome them and receive the care that you need and deserve.
